Specialty
Paediatrics
Almost every paediatric dose in India is arithmetic done at the bedside, on a weight taken this morning, by a nurse covering more beds than the roster admits. The immunisation history that would answer the other half of the consultation is on a card the family carries, and the card is at home.
The dose is arithmetic, and the arithmetic is done at two in the morning.
Impact and action
The dose.
- Milligrams per kilogram are worked out by hand, on paper, per drug, per child, per shift. The calculation is inside the form, with its own validation, and the form is filled once and locked when it is finished.Glyph
- A dose outside the range for that weight is caught by whoever checks it.The hospital defines when the warning fires, and a warning nobody acknowledges escalates up a chain of people.Sentinel
- Whether the third dose of the day was actually given is answered by reading a chart. Each ordered dose is a task with a due time, marked given, missed or skipped, against the person on shift.Quest
The immunisation card.
- The schedule lives on a card the family carries, so a lost card is a restarted schedule and a missed visit is invisible. The schedule is a run of due tasks on the record, and the reminder goes out on the channel the parent chose.QuestRaven
Deterioration.
- Early warning scores are written on a bedside chart, where a rising score is visible only to whoever is holding it.The score is a form with a calculator, and a threshold crossing raises an alert with an owner and a history.GlyphSentinel
The record that has to last eighteen years.
- Growth, allergies, admissions and developmental milestones are spread across whichever hospitals the family used. One record holds the notes, the files, the scans and the completed forms, and every other tool asks it rather than keeping a copy.Atlas

Consent
A child's record is consented to by somebody else.
India now requires that to be verifiable.
The Digital Personal Data Protection Rules name children's data as a category requiring verifiable parental consent, and they run on a fixed clock. A consent captured on a printed form at an admission desk is a consent that cannot be produced later on demand.
So consent is held as state.
Cirith treats consent together with admission as the gate on a record, keeps the complete log of who accessed what, and lets an administrator answer who currently holds access before a change is made.
