Every department, one record
The coordination layer of a hospital.
One admission crosses the emergency room, a ward, a theatre, a laboratory, a pharmacy, a billing counter and an insurance desk. Each of them acts on what the last one knew. Pensieve holds the record they all write to.
The problem
The seam between departments.
A hospital is a set of departments that each work correctly and still fail together. The emergency room stabilises a patient and the ward admits them without the triage note. The laboratory releases a critical value and nobody owns the phone call. The theatre records an implant and the billing counter never sees it. The insurance desk assembles a claim out of four systems, and the deduction that comes back names a document that existed in the building the whole time.
Every one of those departments did its own job. The failure lives in the space between them, and that space has no head of department.
The seam is where the money goes.
A consumable issued on a ward and never billed. An implant opened in theatre and reconciled from memory a week later. A package rate applied against the wrong scheme because the desk that priced the case could not see the desk that admitted it. A claim returned with a deduction because the discharge summary and the operative note describe one admission two ways.
Each of those is a coordination failure wearing a financial costume. A hospital counts them as billing problems and staffs a desk to chase them, which is how a seam becomes a permanent cost centre.
The seam is where the clock goes.
Scarce clinical staff spend the day re-entering what another department already recorded, and patients wait in a queue for a record that exists elsewhere in the same building. The specialist shortage an Indian hospital feels is a real shortage with a clerical load laid on top of it, and only one of those two is purchasable.
Delete the second entry. Pensieve places an order once and carries it to fulfilment, to the dose schedule, to the bill and to the accreditation file without a further keystroke.
The seam is where the record breaks.
Departments buy software the way they buy equipment: one system for the laboratory, another for imaging, a third for billing, a fourth for the scheme desk. Each was bought to solve one department’s problem, and each keeps its own copy of the patient. The copies drift, reconciling them becomes a job, and the job becomes a team.
Eliminate the reconciliation desk. Pensieve holds one identity, one clinical record and one authorization decision for the whole hospital, and every tool reads them rather than keeping a copy.
How a hospital is wired today
Six departments, six versions of one patient.
The mechanism
How the coordination is actually done.
One identity, opened once.
Gate is the front door. An outpatient visit, an emergency case, a ward admission, a day-care visit or a self-referral opens one visit, and every tool downstream attaches to that one. The registration desk stops creating a patient the hospital already has.
One record, and one copy of it.
Atlas is the only tool permitted to hold a patient’s health information. Diagnoses, notes, documents, images and completed forms live there, and every other tool asks Atlas rather than keeping its own copy. That single rule is what makes a fact entered on the ward a fact at the billing counter.
One order, carried to its ends.
Forge holds the hospital-approved catalogue and places the order: a drug, a test, a scan, a procedure, a referral or a form. The same act schedules the doses in Quest, raises the charge in Vault, routes the requisition to the laboratory or an outside partner, and shows a live position on the patient’s timeline.
One decision about who may look.
Cirith checks every read and every write against the person’s role, the patient’s consent and the current admission, and writes the check to a log an administrator can query by patient or by staff member. Emergency access has its own path and is always recorded.
One picture of the building.
Nexus Manager holds a digital twin of the hospital: wards, rooms, beds, equipment, services, teams and duty rosters, kept live by the people who run them. A task lands with whoever is actually on shift because the roster and the task are the same system.
One bridge to what the hospital already runs.
Westron connects the analysers, the imaging store, the accounting package and the systems already in the building, translates their traffic onto the same record, and keeps a forensic trail of every exchange. Nothing is ripped out to let Pensieve in.
How a hospital is wired on Pensieve
The same six departments, one record underneath.
Impact and action
Revenue crosses six desks before it becomes a claim.
- A consumable is issued on a ward, an implant is opened in theatre, a drug goes out at the counter. Each is recorded where it happened, and the bill is assembled afterwards from whatever reached the billing desk.Vault raises the charge at the moment the order is fulfilled, on the same record the clinician wrote to, so the bill is a reading of the care rather than a reconstruction of it.VaultForge
- Claims reach insurers through third-party administrators, and the desk works one portal, one document checklist and one tariff sheet for each of them.Vault carries the payer, the scheme and the tariff on the visit itself, and produces each administrator’s pack from the one record the admission already wrote.Vault
- Government scheme billing is package-rate billing against a national procedure master, submitted through a portal that behaves differently in each state, alongside the separate rate schedules a hospital already runs for central and state employees.Vault applies payer-specific pricing, package rates and hospital-approved pricing rules from one place, and keeps a tamper-proof trail on every transaction.Vault
- The cashless authorisation clock starts at admission, and the documents it needs are held in four systems by three departments.Cadence runs the authorisation as a workflow with a named owner and a visible position on every step, and Sentinel escalates the step that runs late.CadenceSentinel
- A claim is deducted because the discharge summary, the operative note and the investigation report describe one admission three different ways.Atlas holds one record with author, timestamp, department and signature on every entry, so the three documents are three readings of the same facts.Atlas
The staff a hospital cannot hire are doing clerical work.
- A specialist’s day fills with re-entering what another department already recorded, and the shortage the hospital feels is the real one with clerical load stacked on top.Forge places the order once, from a fixed hospital-approved catalogue, and it travels to fulfilment, to the schedule and to the bill without a second entry.Forge
- The nursing handover is spoken, and what was agreed on the morning round is reconstructed on the evening one.Quest turns every due dose and every step into a timed task held against the duty roster, and Glyph makes the shift handover a structured form that locks the moment it is finished.QuestGlyph
- A form a department needs is a change request to a vendor, so the department keeps the form on paper and types the result in later.Glyph lets the department build its own form, with validation, calculators and a formal sign-off, and Durin composes the screen around it, either by your own staff or by a Pensieve engineer working in your building.GlyphDurin
- Registration holds a queue at the outpatient desk while the record it is waiting for already exists inside the hospital.Gate opens one visit that the record, the orders and the bill all attach to, and hands the patient straight on to the tools that carry them.Gate
- Rearranging a ward, adding a service or changing who covers a shift means a call to a vendor and a wait.Nexus Manager holds the hospital’s wards, resources, teams and rosters as a digital twin your own administrators edit, and Cornea rehearses the change as a simulation before it reaches a live floor.Cornea
One patient, one record, one decision about who may see it.
- A patient’s history sits in the outpatient file, the ward notes, the laboratory system and the imaging store, and no two of them agree on who the patient is.Atlas is the only tool allowed to hold a patient’s health information, and every other tool asks Atlas for it. There is one copy to secure, to back up and to answer for.Atlas
- Consent is taken at the front desk and never reaches the laboratory, the pharmacy or the patient’s phone.Cirith treats consent and the current admission together as the gate on a record, checks the pair on every read and every write, and keeps a log an administrator can query by patient or by staff member.
- Accreditation evidence is assembled in the weeks before an assessment, out of registers that were kept for the assessment.Vigil holds the incident register, committee minutes, training and credentialing files, equipment schedules and internal audits live, so the evidence is the exhaust of running the hospital.Vigil
- The analysers, the imaging store and the systems the hospital already runs hold facts the rest of the building cannot see.Westron connects them over one outbound link, translates their traffic onto the same record, and keeps a forensic trail of every exchange.Westron
- The question of who read a record, and on what basis, is answered by a search through five audit logs that were never designed to be read together.Falcon reads from the same governed data every department works on and shows where each number came from, so an answer can be traced rather than assembled.Falcon
Where the money is actually made
A charge raised where the work happened.
Evidence
What coordination is worth, measured by other people.
Three findings from published research on hospitals that had never heard of Pensieve. In each one the failure sat on a department boundary, and the intervention was a single shared account of the patient. Every hospital running Pensieve is under a confidentiality agreement, so we publish no outcome measure of our own, and none of these is ours.
- After nine hospitals put the handover between clinical teams into one structured form, medical errors fell 23 percent and preventable adverse events fell 30 percent across 10,740 admissions. The failure is the one this page is about: what the outgoing team knows reaching the incoming one by memory. The intervention was a single structured account of the patient, spoken and written the same way at every handover. Starmer AJ, Spector ND, Srivastava R, et al. Changes in Medical Errors after Implementation of a Handoff Program. New England Journal of Medicine, 2014;371(19):1803-1812.
- At eight hospitals across eight countries, deaths fell from 1.5 percent to 0.8 percent and inpatient complications from 11.0 percent to 7.0 percent after the surgical team began briefing off one checklist.A theatre is three departments standing in one room. The checklist obliged the surgeon, the anaesthetist and the nursing team to state the same facts out loud at the same moment, before the incision. Haynes AB, Weiser TG, Berry WR, et al. A Surgical Safety Checklist to Reduce Morbidity and Mortality in a Global Population. New England Journal of Medicine, 2009;360(5):491-499.
- Under a state mandate that put emergency sepsis care on one clock, each additional hour taken to complete the three-hour bundle carried higher risk-adjusted in-hospital mortality, at an odds ratio of 1.04 per hour across 49,331 patients at 149 hospitals.Sepsis care is an emergency room, a laboratory and a pharmacy working against one clock. A state regulator made the clock shared, then measured what each hour of drift cost. Seymour CW, Gesten F, Prescott HC, et al. Time to Treatment and Mortality during Mandated Emergency Care for Sepsis. New England Journal of Medicine, 2017;376(23):2235-2244.
