Emergency Department

An emergency department is the one place in the hospital where the record has to be created while the treatment is already happening. Registration, triage, the medico-legal entry and the first order all compete for the same ninety seconds.

Triage bay, resuscitation room, observation, casualty deskRegistration and treatment at the same momentMedico-legal from the first minute

Impact and action

The clock starts before the record does.

  • A collapsed patient is treated for twenty minutes as an unnamed case, and the notes, drugs and samples of those twenty minutes are attached to whichever file is opened afterwards.Gate opens a visit at arrival with a provisional identity, and every order, sample and note from the first minute attaches to that visit and travels with it when the identity is confirmed.Gate
  • Triage is a category written on a card at the door, and the department cannot say later how long a red case waited.Triage is captured on the record at the door, and Falcon reports waiting and treatment time by triage category rather than by anecdote.GlyphFalcon

Time critical protocols are run from memory.

  • Sepsis, stroke, chest pain and trauma each have a protocol on a laminated sheet, and whether the steps happened inside the window is reconstructed afterwards from the case sheet.Cadence runs each protocol as a timed workflow with a visible step, a next step and an overdue flag, so the window is being watched while it is still open.Cadence
  • A patient deteriorating in the observation bay is noticed by whoever walks past.Sentinel raises an owned alert with an escalation chain when the rule fires, and records whether it was seen, acknowledged and resolved.Sentinel

The medico-legal file is assembled months later.

  • A medico-legal case is flagged in a register at the desk. The police requisition arrives in March for a case in November, and the file is rebuilt from three departments. The medico-legal flag is a property of the visit, and Glyph captures the medico-legal entry, the wound certificate and the consent as locked, versioned forms on the record itself.GlyphAtlas
  • Who saw the record, and when, is unanswerable, which is exactly the question a court asks.Cirith keeps a complete access log, including the emergency access path, which is always available and always recorded.

The clock

Door to decision.

  1. Door

A visit opens on arrival. A named patient resolves against the existing identity; an unidentified patient gets a provisional one that is merged later without losing a single order.

  2. Triage

The triage category, the first observations and the medico-legal flag are captured on the record. The clock the department will be measured on starts here, automatically.

  3. Protocol

Cadence starts the run that matches the presentation. Each step shows its owner, its due time and its state, on a board the whole shift can see.

  4. Orders

A fast-track order set places bloods, imaging, drugs and a cross-consultation in one action, and each one is visible from the bay it was ordered in.

  5. Disposition

Admit, discharge, refer or transfer. An admission moves the same visit into a ward, so the ward inherits the resuscitation record rather than a summary of it.

Nothing here is a new step. Each one already happens, and each one is currently timed by somebody remembering to look at a clock.

The tools this department lives in.

Read top to bottom. Each tag is a tool inside Pensieve, and the line beside it is what that tool does in a casualty specifically.

Tools used in an emergency department and what each one does there
ToolWhat it does in this department
GateRapid registration at the door, the provisional identity, the emergency visit, and the move into a ward or theatre.
CadenceSepsis, stroke, chest pain, trauma and poisoning protocols as timed runs, with an overdue flag and a live board.
SentinelDeterioration and critical value alerts with an owner and an escalation chain, grouped and closed as a set.
ForgeFast-track order sets: bloods, imaging, drugs and cross-consultations placed once and tracked to completion.
GlyphTriage form, medico-legal entry, wound certificate, consent for procedures, and the observation chart.
AtlasThe record itself, including any previous attendance, and the imaging viewer for the scan that was just done.
QuestTimed observations and repeat drug doses in the observation bay, landing with whoever is on that shift.
VaultEmergency billing, scheme eligibility and the deposit, without holding up treatment to establish any of it.
FalconDoor to triage, door to decision and door to admission, by category and by hour of the day.

Compliance

The obligations that begin at the door.

An Indian casualty carries a legal load no other department carries, and most of it attaches in the first few minutes.

  • Emergency treatment is owed before any question of payment, identity or scheme eligibility is settled. Registration and billing run alongside treatment here, and neither one gates it.
  • The medico-legal case is a flag on the visit, and the entry, the certificate and the police correspondence are locked forms with a version history, readable years later against the exact form that was signed.
  • Emergency access to a record that consent has never opened is a real clinical need. Cirith provides that path, and it is fully logged every time it is used, which is what makes it safe to have.
  • ABHA linkage is offered and stays voluntary. Nothing in the emergency flow is allowed to wait on it.

The departments a casualty wakes up.

The theatre

An emergency laparotomy needs a theatre, an anaesthetist and a set. The case is booked against the same visit, so the theatre list shows the emergency alongside the elective rather than beside it.

Operating theatre

Critical care

A transfer into intensive care moves the visit. The resuscitation record, the drugs given and the running protocol arrive with the patient.

ICU and critical care

The blood bank

A massive transfusion protocol is a crossmatch order and an issue record under time pressure. Both are the same object, so the count is right afterwards.

Blood bank and transfusion

The ambulance service

A pre-arrival alert from the road lets triage open the visit before the trolley does, which is the only way the door clock ever starts early.

Ambulance and EMS

Delete the reconstruction. An emergency department should be able to answer, months later, what happened in the first ten minutes, because the record was written in those ten minutes.
