Department

NICU

A neonatal unit treats a patient who arrives without a name, without an identifier and without the ability to consent to anything. Everything the unit does has to attach to a record that somebody had to decide to create.

Incubator, warmer, ventilator, feed chartWeight-based dosing at every stepConsent belongs to a parent

The patient who has to be created before being treated.

A baby written into the mother's file is a baby whose paediatric record starts weeks late and whose scheme claim, immunisation and follow-up all inherit that lateness.

In most Indian hospitals the newborn is an annotation for the first days of life. The identity is created when somebody needs one, usually for a discharge summary or a claim, and the record of everything that happened before that moment is copied across by hand.

That single decision, when the record opens, is what determines whether the unit can later answer a question about a specific baby.

Who is actually in the room.

The neonatologist

Reads trends across hours and needs the ventilator, the monitor and the blood gas on one timeline rather than on three charts.

The neonatal nurse

Runs feeds, weight checks, phototherapy and drug doses on intervals measured in hours, for several cots at once, across a shift.

The mother

Is often in a different ward, is the only person who can consent, and is the one asking for an update. Her record and the baby's are two records that must be linked without being merged.

The billing and scheme desk

Needs a claim in the newborn's name, under the newborn's eligibility, assembled from a stay that began before the newborn had a name.

Impact and action

The newborn has no record of their own.

  • The baby is documented inside the mother's file, and a separate identity is created days later, so the first days of care sit under somebody else's name.Gate opens the newborn's own visit and identity at birth, linked to the mother rather than contained inside her record.Gate
  • The scheme claim, the birth notification and the discharge summary are each assembled from a different source, and they disagree.Atlas holds one neonatal record, and the claim, the notification and the summary are three readings of it.AtlasVault

Every dose is a calculation.

  • Neonatal doses are weight based, frequently diluted, and worked out on paper at the cot side at three in the morning.Forge orders from the unit's own neonatal catalogue, and Glyph carries the calculators inside the form so the arithmetic is part of the record rather than beside it.ForgeGlyph
  • Feeds, phototherapy and weight checks run on intervals a nurse holds in her head across four cots.Quest turns each of them into a due task with a time and an owner, and keeps the history of what was given, missed or skipped.Quest

The machines are off the record.

  • Ventilator settings and monitor readings are copied by hand onto a chart, and the chart is the only place the trend exists. The orophin bridge carries neonatal ventilator and monitor output onto the record over one outbound connection, matched to the right cot and the right baby.Westron
  • A deteriorating baby is escalated by whoever is standing at the cot.Sentinel raises an owned alert with an escalation chain, and the unit gets a record of how quickly it responded.Sentinel

Read top to bottom. Each tag is a tool inside Pensieve, and the line beside it is what that tool does in a neonatal unit specifically.

GateThe newborn's own identity and visit at birth, the cot census, and the link to the mother's record.
QuestFeeds, weight checks, phototherapy hours, drug doses and observation intervals as due tasks against the roster.
ForgeNeonatal order sets from the unit's own catalogue, including dilutions, blood products and imaging.
GlyphThe neonatal assessment, the feed chart, weight-based calculators and parental consent with a formal sign-off.
AtlasThe one neonatal record: notes, bridged readings, results, images and the discharge process itself.
SentinelDeterioration and critical value alerts with an owner and an escalation chain, grouped and closed as a set.
RavenUpdates to a mother who is in another ward, on the channel she consented to, with a delivery log.
VaultThe claim in the newborn's name, under the newborn's scheme eligibility, built from the newborn's own stay.

Consent and identity

A record about a person who cannot speak for it.

Everything a neonatal unit records is data about a child, and Indian data protection rules treat that as a category of its own.

  • Consent belongs to a parent or guardian, and it is captured as a signed, versioned form attached to the newborn's visit rather than to the mother's.
  • The link between the mother's record and the newborn's is a relationship, so a permission granted over one is a deliberate decision rather than an accident of the file they happen to share.
  • ABHA creation for a newborn is offered and stays voluntary, and it never gates admission or treatment.
  • Cirith holds the access log, including the emergency path, so the question of who read a child's record has an answer for as long as the record is retained.

The hospital decides who may open a child's record. Pensieve enforces that decision and keeps the proof. Read how access is decided and the compliance file.

A newborn should leave the hospital with a record that begins at birth. Everything else the unit is asked for later is a reading of that record.
