Specialty

Dermatology

Dermatology is a photographic specialty practised on personal phones. The image that proves a lesion changed sits in somebody’s camera roll, outside the record, outside consent, and outside anything the hospital could produce if it were asked to.

Impact and action

The evidence is on somebody’s phone

  • Clinical photographs are taken on a personal device, shared over a consumer messaging app, and stored wherever that device stores things.Glyph captures the image as an attachment on the form that describes it, with the consent step signed in the same act, and Atlas is the only place it then lives.GlyphAtlas
  • Nobody can answer who has opened that image, or on what basis, which is the exact question India’s data protection rules now put to the hospital.Cirith decides every read against the patient’s consent and the admission, keeps the access log, and lets an administrator ask who currently holds access before anything is changed.Atlas

The lesion is compared from memory

  • Progress is judged against a description written six weeks ago by somebody else, in a clinic running at a few minutes per patient.Atlas holds the photographs, the lesion forms and the results as one timeline for the patient, so the comparison is a scroll rather than a recollection.Atlas

A course of treatment is booked one visit at a time

  • Phototherapy and biologic courses run for months on a fixed rhythm, and the hospital books them as a series of unrelated appointments.Quest runs the course as a schedule with due dates against the patient, marks each session as given or missed, and Raven sends the reminder on a consented channel.QuestRaven

The biopsy comes back to the department rather than to the order

  • A histopathology report arrives as paper at a desk, and finding the patient it belongs to is somebody’s afternoon.Forge sends the specimen out as an order and Westron manages the exchange with the laboratory, so the report returns matched to the order that created it.ForgeWestron

In the clinic

A few minutes per patient, and a decision that turns on what six weeks ago looked like.

A clinician talking with a patient in a consulting room, notes in hand.
The comparison that decides the treatment is the one the record is least equipped to hold.

Why the consent question lands here first

A dermatology photograph is identifiable health information about a person who is usually awake, well and standing in a clinic. It is the easiest image in the hospital to take and the hardest to govern, and it is normally taken on a device the hospital does not own.

Putting it on the record changes three things at once. The image acquires an author, a time and a purpose. The consent that permitted it is signed in the same act rather than assumed. And the question of who may open it later is answered by the access system rather than by whoever holds the phone.

Powerful. Responsible. Pensieve.
