Cases are booked into a suite whose capacity, staffing and instrument availability live in three separate books.
The procedure
Findings and scores are dictated, written on a pad, or typed later from memory by whoever is free.
The specimen
A biopsy leaves the room in a pot with a handwritten label, and the record loses sight of it until a report appears.
The result
Histopathology returns days later, to a department rather than to the order, and the patient is telephoned if somebody notices.
Four boundaries, and each of them is currently a re-entry.

The list is a diary and the suite is the constraint
