An Indian blood bank is licensed, inspected and audited on its own registers, and it keeps them separately from the ward that asked for the unit. So the request, the cross match, the issue, the transfusion and the reaction report are five records of one event, reconciled by a person walking between two rooms with a form.

The unit.
