Nephrology
Haemodialysis is the procedure Indian hospitals claim for most often, and it never ends: two or three sessions a week, for years, for a patient who usually travels to reach the chair. The session is scheduled on a whiteboard, recorded on the machine, documented on three paper forms, and billed from a fourth.
A dialysis unit is a schedule that never stops.
Chairs, shifts, machines, dialysers, water quality, serology, transport and a claim: a unit is the same set of decisions taken again every few days for every patient on its list. Run on paper, the decisions are cheap and the reconstruction is expensive, and the reconstruction is what an assessment, an audit and a claim all ask for.
