Physical Medicine and Rehabilitation
Rehabilitation is the one service in a hospital sold by the session and delivered by the hour, and it is the one most reliably under-billed. A plan of twenty sessions leaves the record as a line in a note, and the twenty are counted on a register at the department desk.
A plan is a schedule with a charge attached to every step of it.
The assessment produces a plan. The plan produces sessions. Each session produces a charge and a note. Written down, those are four artefacts in three books, and the one that goes missing is always the charge.
Move past the session register at the desk that is totalled at the end of the month. Quest carries each session as a due task with an owner, and Vault puts the charge on the account as the chargeable order is placed.
Impact and action
The plan.
- A therapy plan is written into a note and repeated verbally at every handover between therapists. The plan runs as a recurring schedule of due tasks, and each one carries the state it ended in.Quest
- Functional assessment scores are recorded on a printed sheet and compared by memory. The assessment is a form with its own calculator and version history, locked when it is finished and held on the record.Glyph
The session that is delivered and never billed.
- A session delivered on a busy afternoon is written on a register and reaches the bill if the register reaches the counter. The charge is the same object as the record of delivering it, and it lands as the order is placed.Vault
The therapist hour.
- Therapists are allocated by whoever is at the desk, and the department learns its own utilisation from a monthly count.Staff, shifts and the rooms they work in are objects in the digital twin of the hospital, so a due task lands with somebody who is actually on duty.

What the department can prove
Function is the outcome, and it is the one thing nobody records twice.
Measured on the same form each time.
Glyph builds the department own assessment, with its validations and its calculators, and keeps the version history, so a score from three years ago still matches the form that produced it.
And read without a separate export.
Falcon reads from every tool without owning or changing any of it, and shows where each number came from, so a claim about improvement resolves to the records behind it.
