Department

Quality / Accreditation Office

Most Indian hospitals reconstruct their accreditation evidence in the six weeks before an assessment, from registers kept by twelve departments that each keep them differently. The file is real, the work behind it was real, and almost none of it was written down at the time.

IndicatorsIncidentsCommitteesEvidence

This office is a budget holder. The accreditation it owns is paid for in the tariff the hospital claims against.

The published record

What accreditation is worth, and what it costs to prove.

An empanelled hospital is paid 10 percent above the base package rate for every approved claim while it holds entry level accreditation, and 15 percent while it holds full accreditation. Source 23 Several states have made entry level certification a condition of empanelment outright. That is a tariff rule rather than an outcome, and it is the reason this office belongs in a revenue conversation.

The burden is standing evidence.

The evidence an assessment asks for is a register of incidents and their investigations, quality indicators submitted monthly, infection rates, medication error and adverse reaction logs, mortality review minutes, credentialing and privileging files per consultant, sterilisation load records and equipment calibration records. None of that is policy writing. All of it is a record of work that already happened.

And the laboratory is counted separately.

Medical laboratories in India are accredited under their own scheme, and the national board publishes the count of them. Source 30 A hospital file and a laboratory file are two obligations in one building.

Impact and action

Indicators compiled by hand.

  • Monthly indicators are collected from twelve departments on twelve formats, and by the time they are consolidated the month they describe is over.Indicators are read from the governed data by Falcon, which reads every tool without owning or changing any of it and shows exactly where each number came from.Falcon
  • A number in the management pack cannot be traced back to the cases behind it, so a challenge to it cannot be answered.Every figure resolves to the records that produced it, and a figure whose data has moved on is flagged as out of date on the screen.Falcon

Incidents that stop at the form.

  • An incident is reported on a paper form, investigated by whoever picks it up, and closed by being forgotten.Incidents run from report through investigation to resolution as tracked work, with the root cause analysis and the corrective action attached to the same record.Vigil
  • Committee decisions live in minutes in a folder, so an action item agreed in March is discovered unstarted in September.Committees, their meetings and their action items are managed in one place, and an overdue item is visible while it can still be closed.VigilCadence

The six-week scramble.

  • The quality cell stops doing quality work for six weeks in order to assemble the evidence that the quality work happened.Vigil watches what the hospital already does and turns it into the evidence an assessment asks for, so readiness is a standing state rather than a project.Vigil
  • Policies are circulated by email and acknowledged by reply, so nobody can prove who has read the current version.Policies are managed with their versions, and acknowledgement is a record against the person rather than an inbox.Vigil

Where the file comes from

The evidence is downstream of the work.

DAILY WORKIncidentsIndicatorsTraining recordsEquipment checksEvidence file
Incidents, indicators, training records and equipment checks are things the hospital does on days when nobody is being assessed. The file an assessor opens is what those four produce, rather than a document written about them afterwards.

The tools this office runs on.

The quality cell owns almost none of the data it is answerable for. Each row is what the tool does for this office, with the example a quality head would recognise.

Tools used by the quality and accreditation office, what each does here, and an example.
ToolWhat it does hereExample
VigilHolds incidents, investigations, committees, training, audits, policies, risk registers and equipment maintenance as standing evidence. The file an assessor asks for is already assembled on the day they ask.
FalconReads indicators from every tool without owning or changing any of them.A monthly indicator resolves back to the cases behind it.
CadenceRuns corrective actions and audits as tracked procedures with owners and due times. An action item from a committee meeting is overdue on a board rather than in minutes.
SentinelRaises an owned alert on the hospital’s own rules and escalates it.A sentinel event reaches the quality head and the medical director together.
GlyphBuilds the audit and self-assessment forms the office uses, locked once completed.A departmental self-assessment that cannot be edited after submission.
AtlasHolds the clinical record every piece of evidence is drawn from.A record review draws from the record itself rather than from a photocopy of it.

Governance

What this office can say to an assessor, and what it cannot.

Compliance is a property of how a hospital works. Software holds the record and produces the evidence; it never makes an institution compliant, and any vendor who says otherwise has moved a liability onto the hospital without telling it.

The evidence is attributable.

Every entry carries author, timestamp, department and signature, and the audit entry is written in the same transaction as the change rather than by a job that runs later. That is what makes an evidence file answerable rather than merely complete.

Access to evidence is itself governed.

Cirith decides per action whether a specific person may take it, and an administrator can look up everything one person can reach. An assessor sees what the hospital decided an assessor should see.

One group, several files.

A hospital group keeps one governance model and one indicator definition across its units, while each unit keeps its own accreditation file and its own assessment. One definition is what makes two units comparable at all.

Powerful. Responsible. Pensieve.
