Department
CSSD
Sterile services is the only department whose entire output is a claim: that this tray, on this day, was processed correctly. The claim is usually recorded in a bound register, and it is tested only when something has already gone wrong.
The loop
Dirty to clean and back to the table.
Receipt
A used set returns from the theatre against the case that used it. The count on arrival is recorded, and a missing instrument is an open item with an owner rather than a note on a board.
Decontamination
Washing and disinfection run as steps with parameters, so the cycle that a set went through is a recorded fact about that set.
Assembly
The set is rebuilt to its own checklist. The person who assembled it, the contents and the packaging are recorded, which is what makes a later count meaningful.
Sterilisation
The load carries its sets, its physical parameters, its chemical indicator and its biological indicator. A failed load names the sets in it.
Storage and issue
The sterile set is issued to the next case. Cycle, load and set travel with it, so the theatre knows what it is opening.
The value of the loop is the last line. A set on a theatre table should be able to name the cycle that produced it.
Impact and action
The register is the system.
- Cycles are written into a bound register at the autoclave, and the biological indicator result is added when it comes back, by whoever is there.Cadence runs the cycle as a tracked workflow with a live state, and Glyph carries the load record and the indicator result as structured, locked forms.CadenceGlyph
- A failed load is discovered late, and identifying the sets it contained means reading back through the register. The load names its sets, so a failure is a recall list rather than an investigation.Cadence
The set and the case are never linked.
- Sets are issued to theatres rather than to cases, so nobody can say which patient a particular tray was opened on.Forge links the set to the case that consumes it, so the question runs in both directions: which sets this patient met, and which patients this set met.Forge
- Instrument loss is noticed at a stock take, months after it happened.Counts at receipt and at assembly are recorded steps, so a shortfall has a date, a set and an owner.Glyph
The evidence file is built in a panic.
- Cycle logs, indicator results, maintenance records and staff competency are gathered into a folder in the weeks before an assessment.Vigil reads the department's own activity as it happens and keeps the file live against the standard, including the equipment and infection control chapters.Vigil
- Autoclave preventive maintenance and validation are tracked in the biomedical department's spreadsheet, and sterile services finds out when a machine is down.Quest runs preventive maintenance and validation as schedules against the equipment itself, so both departments read one plan.Quest
The tools this department lives in.
Read top to bottom. Each tag is a tool inside Pensieve, and the line beside it is what that tool does in sterile services specifically.
| Tool | What it does in this department |
|---|---|
| Cadence | The processing loop itself: receipt, decontamination, assembly, sterilisation and issue as a tracked run with an overdue flag. |
| Glyph | Set assembly checklists, load records, chemical and biological indicator results, locked on completion and versioned. |
| Forge | The link between a set and the case that uses it, and the request for a set against a listed operation. |
| Quest | Autoclave preventive maintenance, validation runs and periodic testing as schedules with an owner. |
| Vigil | The infection control and equipment evidence file, incident tracking, and staff competency records. |
| Sentinel | A failed indicator, an overdue validation or an unreturned set as an owned alert with an escalation chain. |
| Falcon | Turnaround per set, load utilisation, failure rate by machine, and where the theatre list is waiting on the department. |
| Westron | The connection to washer and steriliser equipment that reports its own cycle parameters. |
Published research
What the literature already measured.
Findings about infection and surgical safety, each with its study and a link. None of it is a Pensieve result.
- A systematic review found health care associated infection affecting 15.5 patients per 100 admitted in developing countries, with surgical site infection the most frequent category.Allegranzi B, Bagheri Nejad S, Combescure C, et al. The Lancet, 2011.
- At eight hospitals across eight countries, deaths fell from 1.5 percent to 0.8 percent and inpatient complications from 11.0 percent to 7.0 percent after the surgical team began briefing off one checklist.Haynes AB, Weiser TG, Berry WR, et al. New England Journal of Medicine, 2009. Instrument counts are one of the checklist items.
- WHO guidance on medical equipment maintenance treats a current and complete inventory as the precondition for the programme.World Health Organization. Medical equipment maintenance programme overview, 2011. An autoclave is the department's critical equipment.
Strapped to two departments.
The theatre
A list that changes at eight in the morning changes what sterile services has to have ready by nine. One list, read by both, replaces a phone call and an assumption.
Infection control
A surgical site infection investigation asks which sets, which loads and which staff. The department already recorded all three, so the investigation reads rather than reconstructs.
Biomedical engineering
The autoclave is critical equipment with a validation schedule and a service history, and both departments should be reading the same plan for it.
The recall question
The question this department exists to answer.
A steriliser fails its biological indicator on a Thursday. The last confirmed good cycle was Tuesday. Which patients were operated on with sets processed in between, and where are those sets now?
In most hospitals that question takes days and produces an answer nobody fully trusts, because it is assembled from a theatre register, a cycle register and a memory of who issued what.
When the load names its sets, the set names its case, and the case names its patient, the same question is a query with a defensible answer, and it is answerable in the hour that matters rather than in the week after. That single chain is the whole argument for treating sterile services as a record department rather than as a utility.
