Specialty
Anaesthesiology & Pain
The anaesthetist meets the patient before the operation, stays through it and answers for the night after it, and is usually the last to be given a system that spans all three. The controlled-drug register that governs half the trolley is still a bound book with a signature in it.
Three encounters, three systems, one clinician
The pre-anaesthetic assessment, the intra-operative chart and the post-operative round are the same patient seen three times, and they are almost never the same record.
The assessment is filled in a clinic and filed in a folder that stays there. The chart is written by hand while both hands are occupied. The round happens on a ward that has seen neither document. What is lost between them is exactly the information that would have made the third encounter shorter.
Impact and action
The pre-anaesthetic assessment never reaches the theatre
- Airway findings, comorbidities, grading and the fitness decision are written in a clinic folder, and the theatre works from what the patient can remember on the morning.Glyph builds the assessment as a form with its own validations and grading, auto-filled from what the record already holds, locked on completion and attached to the case that follows it.GlyphAtlas
The intra-operative chart is written while both hands are busy
- Pressures, saturation, gas and drug times are copied off the monitor onto a sheet at five-minute intervals by the person managing the airway.Monitor output arrives through the orophin bridge and Westron holds that connection, so the chart becomes a reading of the machine rather than a transcription of it.WestronAtlas
The controlled-drug register is a book
- Every ampoule of a narcotic is entered by hand, countersigned and reconciled against a stock book, and the audit trail is only as good as the handwriting.Forge holds the movement as the same object the pharmacy issued, Quest records what was given, missed or skipped, and Vigil keeps the register in the evidence set.ForgeQuestVigil
The post-operative pain round is a favour
- Epidural reviews, block top-ups and rescue analgesia depend on the anaesthetist remembering which ward the patient went to.Cadence runs the acute pain round as a repeatable process with steps and overdue flags, and Quest puts each due review on the list of whoever is on shift.CadenceQuest
Theatre time is the scarcest resource and the least measured
- Start delays, turnover and cancellations are discussed at a monthly meeting, from figures each department assembled for itself.Nexus holds the theatres, the equipment and the roster as a digital twin of the hospital, and Falcon reads utilisation and delay from the tools that record them, with the source of every figure shown.Falcon
In the theatre
The view the patient has, and the view the record should have.
