Cataract surgery is the highest-volume operation in India, and most of it arrives in batches: a screening camp, a scheme list, and a theatre that has to turn over a case every few minutes. The lens that goes into each eye is the most expensive item in the room and the one most often missing from the bill.
The shape of the work
Volume is the whole problem, and volume is where per-case administration breaks.
The list arrives from outside
Patients are screened at a camp or referred against a government scheme, and reach the hospital as a list rather than as records.
The workup is a sheet
Refraction, pressure and biometry are written on a workup card, and the lens power that decides the outcome is read off it in the theatre.
The theatre is a production line
A high-volume list leaves no room for a step that takes a minute per case, which is why implant capture is the step that gets dropped.

Impact and action
The camp list and the hospital record are two different lists
- Patients screened outside the hospital arrive as a spreadsheet, and registration retypes each of them on the morning of surgery.Gate registers the patient and opens the visit once, and Vault carries the scheme’s own tariff against it, so the list becomes records rather than a second copy of them.GateVault
The intraocular lens is chosen in theatre and billed from memory
- The lens is selected during the case from a tray, and the model, the power and the serial number reach the bill through a note written afterwards.Forge places the lens as an order at the point of use against the hospital’s approved catalogue, and Vault raises the charge from that same object on the payer’s tariff.ForgeVault
Workup values live on a card
- Refraction, intraocular pressure and biometry are recorded on paper, and the number that determines the lens power is transcribed at least twice before it is used.Glyph builds the workup as a form with its own validations and calculations, auto-filled from the record, locked on completion and attached to the case.GlyphAtlas
Retinal imaging stays with the machine
- Fundus and tomography studies remain on the device or on a workstation beside it, and the clinic prints what it needs.Studies arrive through the orophin bridge with Westron holding the connection, and Atlas opens them beside the order under the same permission check as the rest of the record.WestronAtlas
Retinopathy screening is a promise made at discharge
- A diabetic patient is told to return in a year, and the hospital learns whether they did when they arrive with a complication.Quest schedules the screening as a due task against the patient and Raven sends the reminder on a consented channel, with a delivery log the clinic can read.QuestRaven
