Specialty
Diabetology
Diabetes is managed in the outpatient clinic, complicated in ophthalmology, operated on in surgery, admitted through the emergency department and paid for by the family. It is one disease with five records, and the only one that travels between them is a plastic folder.

Impact and action
The record that has to travel.
- The eye clinic, the foot clinic and the medicine clinic each keep their own file, and the patient carries photocopies between them.Atlas holds diagnoses, notes, uploaded files, scans and completed forms as one record, and decides who may see or edit each part of it.Atlas
The follow up.
- A review date is given verbally at the end of a consultation and survives only if the patient wrote it down. The review is a due task with an owner, and the reminder goes on a channel the patient consented to, with a record of whether it arrived.QuestRaven
- Annual foot and retinopathy screening is recommended, recorded nowhere, and audited never.Screening is a form the department built, locked when finished, and a due task that shows as overdue when the year passes.Glyph
The bill, over a lifetime.
- Consumables, insulin and consultations are billed at three counters against three price lists.A charge lands on the account as its chargeable order is placed, priced on the hospital approved rules for whoever is paying.Vault
One year
What the platform is holding between two annual reviews.
The review is due
A task with a date and an owner, rather than a note in a diary.
The reminder goes out
On the consented channel, in approved wording, with a delivery log.
The visit is opened
One visit on the record that already holds the previous nine.
Screening is recorded
A locked form with its own version history, and the next one scheduled.
The trend is read
Out of the record, with the source of every figure named.
The long record
Twenty years of a disease is an architecture question.
One place holds the health information.
Atlas is the only tool allowed to hold it, and every other tool asks Atlas rather than keeping its own copy. An automated check fails the build when a tool tries to route around that, which is what keeps the rule true after the first year.
And the record stays portable.
Export speaks HL7 v2, FHIR R4 and DICOM, so a hospital that decides to leave takes twenty years of a patient record with it in a form other software can read.
