Billing & Cashiering
An Indian hospital runs a retail cash counter and a credit book at the same time, on the same patient, often on the same day. The loss here is rarely a rate: it is the consumable used at two in the morning that nobody entered, and the discount agreed verbally at the discharge desk.
The counter
Five workflows an Indian billing desk runs every day.
Each of these is ordinary here and absent from software written for another country’s payer system. A product that models none of them makes the counter do the work by hand, on paper, beside the screen.
- The advance, and the refund.A deposit is collected at admission, receipted, drawn against as the stay runs, and refunded or absorbed at discharge. It is part of the visit rather than a separate cash book.
- The estimate, reconciled to the bill.A pre-admission estimate is given to the family and then has to be answerable at discharge. The estimate and the final bill are two states of one object, so the difference between them can be explained line by line.
- The interim bill.A long stay is billed in parts while it is still running, so the family is never handed a single unexpected figure at the end.
- The split payer.Insurance covers part, the family pays the rest in cash at the counter, and both halves belong to one episode. One ledger holds the split rather than two systems agreeing later.
- The mid-stay conversion.A patient admitted as cash turns out to hold cover. The whole stay is repriced against a different tariff retrospectively, and every charge already posted moves with it.
Impact and action
Charges that never arrive.
- A consumable is opened in the ward, an implant is used in the theatre, a drug is issued from the pharmacy. Each was documented clinically. None of the three reached the bill. The charge is created by the fulfilment event itself. Forge places the order and Vault adds the charge the moment a chargeable order is placed, so the thing that happened and the thing that was billed are one object.ForgeVault
- A discount is agreed at the counter by whoever is standing there, and the hospital finds out at month end how much was given away.Discount authority is a rule in the charge master rather than a conversation. Vault applies the hospital’s approved pricing rules, its taxes and its discounts, and records who applied what.Vault
A tariff engine written for somewhere else.
- One clinical event has to produce a cash bill, a corporate credit note, a third-party administrator claim and a government scheme claim, each against a different rate schedule.Payer-specific pricing lives in one charge master, so one event produces every document each payer needs without a separate desk keying each one.Vault
- Tax on a hospital bill depends on the room type, the rate band and whether a drug was dispensed to an inpatient or across the retail counter. Applied by hand, it is wrong monthly.Tax is an attribute of the line rather than a step someone remembers. Vault applies it per line at the moment the charge is created, and Falcon shows the result as a live view.VaultFalcon
The discharge that waits.
- Discharge stalls while the counter chases the ward for what was used, the pharmacy for what was issued, and the consultant for the summary.Vault can hold or clear a discharge on whether the bill is settled, and it clears against a bill that assembled itself as the stay ran.VaultGate
- Consultant payouts are computed off the finalised bill on a spreadsheet, and the argument about them runs for weeks. The finalised bill is governed data with a tamper-proof trail behind every transaction, and Falcon reads the payout view from it rather than from an export.VaultFalcon
Where a charge is created
Four rooms, one ledger, nothing re-entered.
The published record
Whose money this is.
Households still pay 39.4 percent of India's total health expenditure directly, down from 64.2 percent a decade earlier. Source 24 That single share is why an Indian billing counter behaves as it does: it is a retail counter facing a family, at the same moment as it is a credit desk facing a payer, and the same episode has to satisfy both.
The bill is also the claim’s first document.
In a 2025 study of the national and state assurance schemes in one Indian state, settlement ran from three months to a year against a thirty day guideline. Source 11 A claim built from a bill that assembled itself as the stay ran starts that clock complete. A claim built from a bill reconstructed at discharge starts it with a query.
The tools this counter runs on.
These tags appear on the screens a cashier and a billing executive use. Each row is what the tool does at this counter, with the example they would recognise.
| Tool | What it does here | Example |
|---|---|---|
| Vault | Holds the charge master, the tariffs, the deposits, the invoices and the financial audit trail. | A mid-stay payer change reprices every charge already posted against the new tariff. |
| Forge | Creates the chargeable event, and locks in exactly what was ordered at the moment it was ordered. | An implant used in theatre posts to the bill because the order was fulfilled. |
| Gate | Owns the visit the bill belongs to, and the discharge the bill gates. | Discharge is cleared once the bill is settled, from the same screen. |
| Falcon | Reads the ledger without owning it, and shows where each number came from. | Revenue by payer and by department, with the source of every figure visible. |
| Glyph | Runs the forms the counter is answerable for, locked once completed. | A tariff consent for a scheme package is signed off and cannot be altered afterwards. |
| Raven | Sends the statement and the reminder on an approved template, with a delivery log. | An interim bill reaches the family before the discharge conversation rather than during it. |
Interdepartmental
Every leak has a room it started in.
- The ward. The largest single source of uncaptured charge in an Indian hospital is the consumable used at the bedside. It posts when the nurse records it, on the same record.
- The theatre.Implants and theatre time are the highest-value untracked items in most hospitals. They are captured at the point of use.
- The pharmacy. The same drug is taxed differently depending on whether it was dispensed to an inpatient or sold across the counter. The dispensing context decides, and the context is on the record.
- The insurance and scheme desk.A deduction is usually a documentation failure rather than a pricing failure. The pack the desk submits is assembled from the same ledger the bill came from.
Governance and security
The counter a hospital audits first.
Cash, discounts and write-offs are where an Indian hospital’s internal audit starts, and the question it asks is always the same: who did this, and when. The hospital sets the rule. Pensieve enforces it and records that it was enforced.
The trail is written with the transaction.
Vault keeps a tamper-proof financial trail for every transaction, and the audit entry is written in the same transaction as the change rather than by a job that runs afterwards. A reversal is a recorded event with an author.
Authority is a permission, and it is reviewable.
Cirith decides per action whether a specific person may take it. An administrator can look up everything one cashier can do, and simulate a change to that before making it. Where a hospital wants a counter rehearsed before it goes live, Cornea runs the configuration against a simulated site first, so the mistakes are found where they cost nothing.
The books stay portable.
Invoices and the ledger export in forms an accounting package reads, and the record exports in HL7 v2, FHIR R4 and DICOM. A hospital that decides to leave takes both with it.
