Nursing Services

Nursing holds no budget and can end any rollout by declining to use it. Every minute of documentation added to a shift is contested, and rightly: it is taken from the only part of the day that is genuinely clinical.

WorklistMedication recordHandoverCharge capture

Delete the screen that asks a nurse to record what she has already recorded. A due task, its completion and the charge behind it are one action.

Impact and action

The medication round.

  • Administration is signed on paper at the bedside and entered into a system later, so the record of what was given lags the giving of it by hours. Each ordered dose is a scheduled task with a due time, marked given, missed or skipped at the bedside, and that history stays with the patient.QuestForge
  • An interaction or an allergy is caught at the round rather than at the order.Interaction and allergy rules fire at order time as owned alerts, before the drug reaches the trolley.Sentinel

The handover.

  • The shift handover is spoken, and what survives it is whatever the incoming nurse wrote down. The handover is a structured form the hospital designed, filled once and locked when it is finished, on the same record. [Source 5](/research#ref-5)Glyph
  • A care plan lives in a folder at the nurses’ station, so nobody outside the ward can see where a patient is in it.Care plans run as repeatable procedures with steps, owners and due times, and a step running late is flagged.Cadence

The charge nobody made.

  • A consumable is opened at the bedside at two in the morning and reaches no bill, which the hospital later calls shrinkage.Recording the item against the patient is what posts the charge. One action, at the point of care.ForgeVault
  • Nursing observation is written on a chart at the end of the bed and read by nobody upstairs.Flowsheets are captured on the record, and a deteriorating score raises an owned alert with an escalation chain.GlyphSentinel

The published record

What the literature says about this shift.

Across 300 hospitals in nine countries, each additional patient added to a nurse's workload was associated with a 7 percent increase in the odds of an inpatient dying within thirty days of admission. Source 26 Documentation load is workload, which is why any system that adds keystrokes to a round is a clinical decision rather than an administrative one.

The handover is measurable.

When nine hospitals put the handover between clinical teams into one structured form, medical errors fell 23 percent and preventable adverse events fell 30 percent across 10,740 admissions, with the handover itself taking no longer. Source 5

The tools a ward nurse actually touches.

Each row is what the tool does at the bedside, with the example a ward sister would recognise.

Tools used in nursing services, what each does here, and an example.
ToolWhat it does hereExample
QuestTurns each ordered dose into a due task and shows each nurse her own list.A round opens as a list of what is due, in time order, for her patients.
GlyphRuns the ward’s own forms and flowsheets, locked once completed. The shift handover form the hospital designed, filled once and unalterable afterwards.
CadenceRuns care plans and ward checklists as tracked procedures.A pressure-area care plan whose overdue step shows on the sister’s board.
SentinelRaises an owned alert on the hospital’s own rules and escalates it up a chain.A deteriorating observation score reaches the duty registrar with an owner attached.
ForgeCarries the order and the consumable used against it.A dressing pack recorded at the bedside posts to the bill and draws down stock.
AtlasHolds the record everything above lands on, with author, timestamp and signature.Every entry on the shift is attributable, without a second system.
