Procurement & Stores

Materials are roughly a third of what an Indian hospital spends and the least instrumented third. The store buys against habit because habit is the only demand signal it has, and the cost of that shows up as expiry at one end of the shelf and a stockout at the other.

IndentsReceiptConsumptionReorder

A store cannot order against demand it cannot see. Consumption is the demand signal, and it is created three floors away.

Impact and action

Indents on paper.

  • A ward raises an indent on a pad, the store fills what it has, and the difference between what was asked for and what was issued exists nowhere. The indent is a step on a tracked process with an owner and a due time, and a step running late is flagged on a live board while it can still be fixed.Cadence
  • Receipt against a purchase order is reconciled by hand at month end, so a short delivery is discovered long after the supplier could be challenged.Receipt, the purchase and the consumption that follows feed the same ledger, and Falcon reads spend from it without owning it.VaultFalcon

Consumption nobody linked.

  • What left the shelf and what was used on a patient are two different records kept by two different people.Consumption is the fulfilment event. Forge places the order, the issue draws the stock down, and the same event posts the charge.ForgeVault
  • A high-value implant is issued to a theatre and traced afterwards by asking who remembers. The item is linked to the case that used it, so traceability is a property of the order rather than a search.Forge

Expiry and stockout together.

  • The same store carries a shelf of stock about to expire and a critical item that ran out on Sunday night.Batch, expiry, reorder level and consumption pattern are one live view, so the two failures are visible as what they are: the same missing signal.Falcon
  • A supplier fails and the ward finds out at the bedside.A reorder level breached raises an owned alert with an escalation chain, delivered on an audited channel.SentinelRaven

The published record

Where the money on a shelf actually sits.

An inventory audit at an Indian national institute's pharmacy store found roughly 70 percent of the annual drug budget concentrated in about 10 percent of the items on the shelf, with a separate and only partly overlapping set carrying the items whose absence stops treatment. Source 29 The study names stockout, expiry and supplier failure as the recurring failures, and habit rather than demand as the cause of overstocking.

The same finding holds for equipment.

International guidance on medical equipment maintenance treats a current and complete inventory as the precondition for the whole programme, because inspection, preventive maintenance and repair are all planned against it. Source 18

The tools this store runs on.

Each row is what the tool does in the store, with the example a storekeeper would recognise.

Tools used in procurement and stores, what each does here, and an example.
ToolWhat it does hereExample
CadenceRuns indenting, approval and receipt as a repeatable procedure with a live board. An indent waiting three days for approval shows as overdue to the manager.
ForgeCreates the fulfilment event that draws stock down and posts the charge. An implant issued to a theatre case is linked to that case.
FalconReads consumption, spend and reorder positions from the governed data.Consumption by item and by ward, with the orders behind each figure visible.
SentinelRaises an owned alert on a rule the hospital wrote, and escalates it.A reorder level breached escalates before the ward notices.
VaultCarries purchase and consumption into the ledger.Materials spend reconciles to the same books the revenue does.
DurinBuilds the store app where no system exists, with no code and no vendor ticket.A goods-receipt screen built by the materials manager in an afternoon.
