An endocrine record is a long argument with a moving number. The insulin dose that is right this week is decided from readings taken at home, on a machine the hospital never sees, written on the back of an old prescription.
Titration is a schedule, and a schedule can be run.
The clinic decides a dose. The patient carries it for six weeks. Somewhere in those six weeks the readings drift, and the drift is visible only to whoever is holding the meter.
