Dietary & Kitchen

A therapeutic diet is a clinical order that is delivered by a department with no clinical system. It is written on a round, copied onto a slip, carried to a kitchen, and changed three times before the trolley leaves.

Diet office, production kitchen, ward trolleyThree services a day, every dayA therapeutic diet is a prescription

The order that stops being an order at the ward door.

Every other clinical instruction in the hospital travels as an object. A diet instruction becomes paper the moment it leaves the round.

A renal diet, a diabetic diet, a nil by mouth before a theatre case and a nasogastric feeding regime are all clinical decisions with clinical consequences. In most hospitals they are communicated by a slip, and the kitchen is producing against a count rather than against a list of patients.

The result is that a diet change made at eleven reaches the trolley at one, or misses it entirely, and the record of what a patient was actually fed is a tally rather than a fact.

Impact and action

The diet order is a slip.

  • The diet is written on the round and copied onto a slip that the ward sends down. A change after the slip has gone is a phone call, and a nil by mouth that arrives late is a cancelled operation.Forge places the diet as an order from the hospital's own diet catalogue, routed to the kitchen on the same path every other order travels.Forge
  • The kitchen produces against a headcount and the ward serves against a memory of who is on what. The kitchen reads the live list of diet orders by ward and by bed, so production and service are working from the same object.ForgeGate

Nutrition is never on the record.

  • Screening on admission is a box on a form that is rarely filled, and a dietitian's plan lives in a separate file the ward never opens.Glyph carries nutrition screening and the therapeutic plan as structured forms on the patient record, so the plan is where the round is.GlyphAtlas
  • Intake, tube feeds and supplements are recorded on a chart at the bedside and summarised, if at all, by hand.Quest runs feeds and supplements as due tasks with a time and an owner, and keeps the history of what was given, missed or refused.Quest

Food cost is unattributed.

  • Catering cost is a monthly figure with no line back to a ward, a diet type or a patient day.Falcon reads consumption against diet orders and patient days, so cost is attributable to a ward and a diet rather than to the kitchen as a whole.Falcon
  • Chargeable items such as a special feed or a supplement are consumed on the ward and never reach the bill.A chargeable diet item is an order, so Vault adds the charge in the same movement that the ward consumes it.Vault

The tools this department lives in.

Read top to bottom. Each tag is a tool inside Pensieve, and the line beside it is what that tool does in a hospital kitchen and diet office specifically.

ForgeThe diet order itself from the hospital diet catalogue, including nil by mouth, therapeutic diets and supplements.
QuestFeeds, tube feeds and supplements as due tasks with a time, marked given, missed or refused against the roster.
GlyphNutrition screening on admission, the dietitian assessment, and the therapeutic plan as structured forms.
GateThe ward and bed map the kitchen produces against, and the admission or discharge that changes the count.
AtlasThe record the plan and the intake history sit on, so the round sees nutrition beside everything else.
CadenceThe daily production and service run, and the food safety checks along it, as a tracked workflow.
VaultCharges for chargeable diet items and special feeds, raised where they are consumed.
FalconConsumption by ward and diet type, cost per patient day, and waste against production.
VigilFood safety checks, temperature records and the kitchen's share of the accreditation evidence file.

The department the whole hospital eats from.

The wards

A diet change on the round should change the trolley. When the order is one object, the ward and the kitchen stop reconciling two lists of the same patients.

IPD and inpatient wards

The theatre list

A nil by mouth instruction that arrives late cancels an operation and costs a theatre slot. It is an order against the case, so both departments see it.

Operating theatre

The dialysis unit

A renal diet is prescribed by one department and delivered by another, on a schedule that has to match the session.

Stores

Pensieve integrates with the inventory system a hospital already runs. Where there is none, the app is built in Durin, either by hospital staff with no code and no vendor ticket, or by a Pensieve forward-deployed engineer who builds it for the hospital and hands it over. Anything built that way sits on the same permissions and the same audit trail as the rest of the platform.

Procurement and stores

Food safety

The file the kitchen has to keep.

A hospital kitchen in India is a licensed food business as well as a clinical department, and the evidence it has to keep is the same shape as any other quality evidence.

  • Temperature records, cleaning schedules, pest control and supplier checks run as tracked workflows in Cadence, with an overdue flag rather than a signature added at the end of the week.
  • Staff health and food handler training records are competency records, and Vigil keeps them beside the rest of the hospital's.
  • A complaint or an incident is tracked from report through investigation to resolution, in the same register the rest of the hospital uses.
  • Allergen and therapeutic restrictions are properties of the order, so a substitution at the trolley is a decision that has to be recorded rather than one made quietly.
